WE WANT TO HEAR FROM YOU!Have you been to the clinic in the past few months? If so, we'd appreciate you taking a few minutes to complete a short survey to tell us about your experience. You can access the survey by clicking here. Your responses are anonymous, but they are invaluable in helping our team's ongoing efforts to improve the quality health care you receive.
In addition, we'd also like to hear from you if you've had a telephone appointment with one of our providers. Please find that survey, which takes just a few minutes to complete, here.
ASSESSMENT CENTREOur clinic is proud to be a partner in the Brockville Assessment Centre. Any community member experiencing non-emergent, flu-like respiratory symptoms can attend at the Centre (located in the Brockville Shopping Centre - 125 Stewart Boulevard) to be seen by a medical professional, who can provide a full range of services. Avoid a wait in the hospital emergency department by calling to book an appointment at 613-704-1441 or online here.
HOW TO GET YOUR COVID-19 VACCINEThe Leeds, Grenville and Lanark District Health Unit is no longer holding Community Clinics. Going forward, patients can attend COVID vaccination clinics being held at Health Unit offices throughout the region. To schedule an appointment, phone the Health Unit's Call Centre at 1-844-369-1234 (Monday to Friday from 8:30 a.m. to 4 p.m.)
