• Missed appointments with less than 4 hours notice - up to $75
• Invalid health card at appointment - up to $100
• Medical records request - $30 for under 30 pages .25 cents/page after 30
• MTO Drivers Medical with Form - $200
• Employment/Education Medical with form - $150 
• Fit work work/school etc. note - $40
• Sick note work/school or Request for Prescription for a service - $30
• EI Medical Form - $45
• TB Skin Test - 1 step $30, 2 step $50
• Liquid nitrogen treatment tray fee - $30
• Removal of non-cancerous lesions - $30-$200

We understand that unexpected incidents can occur and you may not be able to attend your doctor's appointment.  Please advise our office as soon as possible if you cannot attend your scheduled appointment.  If a patient has missed (NO SHOW) two visists in a 12 month period a fee will be charged of up to $75 to the patient and this fee must be paid prior to any new appointment bookings.
